Office Policy & Fees

Patient Billing

Payments can be made by cash, check or credit/debit card. For your convenience we accept Visa, MasterCard, Discover, American Express and CareCredit. We deliver the finest care at the most reasonable cost to our patients. Payment is due at the time service is rendered. If you have questions regarding your account, please contact us at 801-498-7668. Many times, a simple telephone call will clear any misunderstandings.

The fee for your endodontic treatment will be based on the specific tooth involved, the type of treatment necessary, and the extent of treatment. During your first visit we will discuss the options available to you, the relative success rates, the probable number of visits, and the fees involved.

For patients who have dental insurance, we will submit any claims for you to your insurance carrier. We require any co-payments and deductibles to be made at the time our services are rendered. In order to do this, we obtain an estimate of the amount your insurance will cover. We have found that some insurance companies do not cover all expected dental services. We will work diligently to ensure that you get the most out of your insurance plan. Most insurance companies will respond within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. Please call our office at 8001-498-7668 for more details.

If You Do Not Have Insurance

You will be expected to pay the full amount at the time of service.  However, we offer interest free payment plans.  For those without dental insurance needing to make payments, a minimum payment of 40% of the fee is required at the time of service. 50% of the remaining balance is due within 30 days and any remaining balance due within 60 days. However, a 10% credit adjustment is applied if payment is made in full at the time of service.
